   To consult with one of our volunteer attorneys, the Disclosure Statement must be read and signed.

   Please read carefully and check each box. Once you have read through the Disclosure, print, date and sign on the bottom. All boxes must be checked for you to speak with an attorney.

   
   

4. Disclosure Statement

   1. I understand that I remain responsible for all aspects of my legal matter.
   2. I understand I am meeting with a volunteer attorney. Any information I provide to the volunteer attorney will be kept confidential.
   3. I understand that the volunteer attorney is providing limited advice and information and does not represent me in my case.
   4. I have the option to hire the volunteer attorney (or any other attorney) but if I retain an attorney it would require me signing a retainer agreement outside of today’s meeting.
   5. I understand that library staff may be present at the clinic to aid in the process and that person will not be providing legal advice.
   6. I understand that the Washington County Law Library is hosting the clinic and that they do not endorse the volunteer or is not liable for the legal advice or assistance given.
   7. I have read and understand this disclosure statement and I agree to its terms.
