Leaving instructions about your health care choices helps ensure decisions are made that are based on your values and preferences, even if you are unable to make your wishes known. This presentation will discuss what advance care planning is and what health care directives can do. We explain who can be a health care agent and the responsibilities of that role. Examples of health care directives will be shared. We will highlight where people can find help completing the health care directives and things to consider when completing a directive.
